About this event
The Minnesota Department of Health and community partners are in the process of creating the first joint Cardiovascular Health and Diabetes 2030 Statewide Plan for Minnesota. This plan prioritizes community input by ensuring your insight will be a core part of how the plan is created and implemented. People from your organization are invited to participate in the co-creation of this new approach to addressing heart disease, stroke, and diabetes in Minnesota communities.

These virtual sessions will create a convening space for groups working on heart health or diabetes prevention, disease management, and treatment within communities faced with the greatest disparities.
Participants will:
- Provide feedback on the plan mission/vision, strategies, and outcomes drafted to date
- Add shape and insight into how communities, organizations, and state agencies can co-create a structure to move and measure collaborative work going forward
Session Information:
There are two virtual sessions being offered. You do not need to attend both. Please select from one of the options listed below:
- Tuesday, July 27, 9 a.m. to 10:30 a.m. online/virtual, OR
- Thursday, July 29, 1 p.m. to 2:30 p.m. online/virtual
Don't want to attend virtually? There is an in-person session on Monday, August 2, 2:30 p.m. to– 4 p.m. at the Minneapolis American Indian Center. This session is for those who would prefer to attend in person rather than virtually. A separate Eventbrite registration page has been set up for that meeting.
All virtual and in-person sessions are 90 minutes. Participants will receive gift cards for their time.
